Le Blanc Consulting provides Supportive Living Services and In-Home Support Services to individuals that are challenged by age, physical ability, and/or development ability and who choose to live at their greatest level of independence. These services can be provided to clients in the counties of Alameda, Contra Costa, San Francisco, San Mateo and Marin. Our approach is to serve the whole person through quality services and customer satisfaction. We will do our best to meet your expectations.
Our In-Home Supportive Services
Le Blanc Consulting services are tailored to the individual needs of each client. After an assessment is complete and the development of an individual care or service plan, a client may receive any of the following services:
Personal Care
Professional, private care to maintain physical cleanliness, health and vitality
•Personal care assistance (hygiene, bathing, toileting, and/or dressing)
•Meal planning and food preparation/cooking
•Medication administration and monitoring
Home Assistance
Caring home support to ensure a comfortable and serene living environment
•Daily living assistance (managing appointments, transportation support, problem resolution to daily life concerns...)
•Household maintenance and organization (cleaning, home organizing, housing assistance
•Money management (budgeting, banking, managing credit, paying bills, financial literacy)
Case Management & Care Planning
Individual Care Plan designed by you, your circle of support and our team of professional care planners to ensure your needs are met
Life Enhancement
Quality life-support services through counseling, referrals and resources
•Transportation coordination and mobility training
•Habitation training and support
•Assistance acquiring and using adaptive devices and assistive technology
•Relationship support (assisting healthy linkages to family, friends and community)
•Education, Vocation and Career support if needed
•Attendant Management Workshops and Training

- Housing (Assisting individuals with identifying what their personal needs are in a home, housing search, identifying generic resources for assistance with rents, housing deposits, furnishings, moving assistance, etc…)

- Roommates or Live In Care(determining whether to have roommate, assistance in finding roommates, assistance in relationships with roommates)
- Household Management Workshops and Training

It's easy to get your services started. Simply call or email today. We'll be happy to talk about your unique needs and establish a customized plan of care. If Le Blanc Consulting beleive we have the capacity to provide appropriate services, a support plan and personalized budget are created with input by all members of the individual's Circle of Support (friends/family/ supporters). The plan includes a proposed timeline, a transition calendar, and specific information about the individual being served.Once the completed proposal is reviewed by the individual and his/her Circle of Support, the completed assessment is delivered to the Regional Center for approval, and a start date to begin providing services is established.
